Every year, Cadence Health finds new ways to improve the health of you and your neighbors. Through community health needs assessments and other planning efforts, Cadence Health identifies the strongest needs in our communities, and invests millions of dollars back into local communities through programs such as:
Access to care for low-income and uninsured patients is a growing issue in DuPage and Kane counties. To better ensure low-income county residents can afford and receive necessary health care, Cadence Health provided an operational grant of $987,000 in 2014 to help fund Access DuPage and other community efforts serving those in need.
Community-Based Heart Failure Program
This program targets patients who have been hospitalized with heart failure and, once released, will care for themselves at home. Its goal is to offer patients education and guidance for the first 90 days after discharge, helping them be active participants in their own care and lowering the chance of being readmitted to the hospital.
Community Outreach Programming
A variety of outreach programs are available to promote healthy living, such as classes and educational courses for healthy eating, parenting, smoking cessation and babysitting instruction. Download the Cadence Health Program Calendar .
ThinkFirst Injury Prevention Program
For more than 10 years, Cadence Health has sponsored a program that teaches children—from kindergarten through high school—how to prevent injuries and think safety. Thought-provoking, upbeat programs are offered at schools and community groups, with hundreds of free bike helmets and booster seats given to those in need. Learn more about ThinkFirst Illinois*.

When the Kane County Health Department announced in 2009 that its child immunization program was in grave danger of losing funding, Cadence Health embraced the opportunity to meet this critical need.
